DOT Mock Specimen Collection

THIS IS FOR MOCK COLLECTIONS ONLY. PROOF THAT THE COURSE WAS TAKEN BEFORE MOCK COLLECTIONS WILL BE REQUIRED AND CHECKED. The mock collections will include the following scenarios: Two uneventful collections. One collection in which the quantity of specimen is not sufficient. One collection in which the temperature of the specimen is out of range. One collection in which the donor refuses to sign either the donor certification on the Medical Review Officer's copy of the CCF or refuses to initial the security strips. Supplies needed for DOT Specimen Collector Mock Collections: 8, Federal Custody and Control Forms 6, DOT-approved split specimen collection kits Warm water (to serve as the specimen) A donor or subject (the DOT requires you to perform mock tests on a live subject) Disposable gloves If you do not have your own supplies, please order them as soon as possible. Your 5 mock collections cannot be completed without these supplies. Supplies can be provided for an additional fee.
